Probably you have more than one Copier EA per one platform. For example, you may have SourceEA on 2 or more charts of the Source platform or ReceiverEA on 2 or more charts of the Receiver platform.
Please remove all excess EA instances and restart the MT4 platform. 
Please note that Forex Copier EA is installed on one chart but copies orders for all currency pairs by default.
